Submission of this job worksheet constitutes a contract between Kelley & Sons Heating & Cooling, Inc. and the builder, as stated, for Kelley & Sons to provide the materials and labor as listed. Once Kelley & Sons Heating & Cooling, Inc. has finished our work, complete payment is expected within 30 days. After thirty (30) days, a time-price differential of one and one-half (1½%) percent per month will be added to the outstanding balance. The purchaser will also be responsible for attorney fees, costs and other expenses of collection incurred by Kelley & Sons Heating & Cooling, Inc. in collecting on this contract. If complete payment is not received within 90 days pursuant to Section 111 of the Michigan Construction Lien Act a Claim of Lien will be filed against the job location address
